Seagrape Health is a private, membership-based medical practice focused on preventive, personalized care. Led by a board-certified physician, we prioritize time, continuity, and evidence-based medicine to help you understand your health and take action before problems become disease — and we also help manage chronic disease. We look at the whole person, including lifestyle, lab trends, risk factors, and long-term health strategy.

Seagrape Health operates as a private, membership-based medical practice. Rather than billing insurance for each visit, patients pay a flat monthly membership fee that covers ongoing physician access, preventive planning, and chronic disease management. There are no copays, no surprise charges per visit, and no claim submissions for the care we provide directly.

Your existing health insurance is not used for Seagrape Health membership services, but it remains important for everything outside of our care: labs, imaging, specialist visits, prescriptions, urgent care, and emergency services. Membership dues may also be eligible for payment through a Health Savings Account (HSA).

This model allows us to keep patient volume low and allows more time with your physician, so you receive thoughtful, personalized care. It is an investment in a long-term medical relationship focused on keeping you well.

Seagrape Health offers longer visits, advanced diagnostic testing, and direct access to a board-certified physician. Care is designed to understand your full health history, monitor trends over time, and create a clear preventive strategy. The focus is continuity and long-term health.

Seagrape Health is designed around preventive and chronic condition care, which does not require a physical clinic for most people most of the time. The most important drivers of health are history, trends, labs, lifestyle, and follow-up — not a quick in-office exam once a year. When an in-person exam is necessary, we offer home visits and will introduce limited in-person options.

Seagrape Health is primarily delivered through virtual visits and is available to patients throughout South Florida. We offer limited home visits for members in Palm Beach County, Broward County, and Miami Dade County for an additional fee. Select in person availability will be introduced for patients who prefer a physical setting.

Your membership includes direct access to a board-certified physician, longer medical visits, preventive health planning, and ongoing follow-up. It covers comprehensive evaluations, advanced lab review, personalized care plans, and continuity of care throughout the year. You are not paying per visit — you are investing in an ongoing medical relationship focused on prevention and long-term health.

Seagrape Health membership is not a replacement for insurance. Your insurance can still be used for labs, imaging, prescriptions, specialist care, and emergencies. Additionally, membership dues may be covered by a Health Savings Account (HSA).

Traditional insurance covers primary care visits, but often not the time, continuity, and proactive prevention you need. Most traditional practices operate under time constraints and high patient volume — visits are brief and focused on immediate concerns.

Seagrape Health serves patients at every stage — whether you're managing chronic conditions and working to prevent complications, or focused on primary prevention and long-term health optimization. Our approach is proactive. We address current conditions while building a strategy to reduce future risk.

Home visits are available for members only. They are offered in Palm Beach County, Broward County, and Miami-Dade County for an additional fee. Membership is required to access ongoing care and home visit services.

No There are no copays. Your membership covers your visits and ongoing physician access. Home visits, when requested, are available at an additional cost.

Seagrape Health does not bill insurance. Seagrape Health Membership is not a replacement for insurance — your existing coverage can still be used for prescriptions, imaging, specialist visits, urgent care, and hospital or emergency room services. Depending on your plan, you may use HSA or FSA funds to pay for membership fees. We recommend confirming eligibility with your benefits administrator.

Many patients with a Health Savings Account choose to use their HSA funds to pay for membership fees. Seagrape Health membership fees may qualify as an eligible medical expense. However, HSA eligibility is determined by your individual plan and administrator. We recommend confirming coverage details directly with your HSA provider or benefits administrator.

If an in-person exam is necessary, we coordinate appropriate local care or refer you to a trusted setting in your area. For members in Palm Beach County, Broward County, and Miami-Dade County, home visits are available for an additional fee.

For many preventive and chronic health concerns, a physical exam alone provides limited information. Conditions like fatigue, metabolic health, cholesterol, blood sugar, sleep issues, and cardiovascular risk are better evaluated through:

  • Labs and trends over time
  • Symptom patterns
  • Lifestyle and environmental factors
  • Ongoing follow-up

When a hands-on exam is truly needed, we arrange a home visit or guide you to the appropriate setting.

Seagrape Health is not meant to replace emergency rooms or urgent care. For emergencies or acute injuries, patients should use:

  • Emergency services when appropriate
  • Local urgent care centers for immediate issues

Our role is to reduce the need for urgent care by managing your health proactively.

Yes, and it's often better. Virtual care removes transportation barriers and long wait times while allowing longer, focused visits. It supports consistent follow-up and clearer communication. When physical evaluation is needed, we coordinate appropriate local care. Home visits are available for members in select South Florida counties for an additional fee.

If you would like to learn more about our services, please schedule a call using the Schedule button on the website. Members can communicate directly through the Seagrape Health portal. Members are also provided with a direct phone number to their physician for ongoing care and support.

Seagrape Health is available Monday through Friday, 8:30 AM to 5:00 PM. For urgent needs outside of regular hours, members may text or call their physician directly. For emergencies, please call 911 or go to the nearest emergency room.

We accept credit card, ACH bank transfer, and cash if desired during in-person visits. Membership can be paid on a monthly, six-month, or annual basis. Patients who pay for six months receive the equivalent of half a month off. Those who pay annually receive one full month off. Longer commitments offer the best value and ensure uninterrupted care.

After your membership payment is completed, you will receive two emails from Seagrape Health confirming your enrollment. One of those emails will include your unique invitation code. Download the mobile app from the App Store or Google Play, then register using that code. Once registered, please complete your intake forms inside the app before scheduling your first appointment. If you do not see the emails, please check your spam folder or contact us for assistance.

Yes, we strongly recommend completing your medical history and intake forms before your first visit. Providing detailed information in advance allows your physician to review your history thoroughly and use your appointment time more efficiently. It helps us deliver a more comprehensive and focused initial evaluation.

We do not bill insurance for membership services. However, we request your insurance information so we can properly process lab orders, imaging, or prescriptions when applicable. Your insurance may be used for those services outside of your membership.

Home visits become available after your initial 60-minute virtual appointment is completed. The first visit allows us to establish your medical history and determine whether a home visit is appropriate. Once your initial visit is finished, additional appointment options — including home visits for eligible members in select counties — will appear in your portal.

All appointment confirmations, reminders, and portal notifications are sent directly from Sigma MD. Please check your inbox and spam folder for emails from Sigma MD.
